If you are interested in our TeleHealth Services, you would download this Health Intake Form. 

1. I would like you to take four clear photos of your tongue from the tip to the back of your mouth. Photo should be as clear as possible. 
2. "Don't take it from side to side." 
3. Once you have captured the pictures, along with the Health Intake Forms (11 pages) you will email them. "info@healenartswellness.com"
4. Once photos are recieved, you will get an email reply, at which time you can come back to this website and schedule your Zoom type appointment ( Doxy.me a HIPAA protected platform.)
